I heard a story with several inaccuracies the other day. Basically, the person said Vuky died in the '66 500 when a kotter pin failed and killed him.
Obviously, Vuky died in an accident in '55 because of lapped traffic. To me, the story sounded a bit like Parnelli in Silent Sam. But I was wondering if someone else in Indy history had died from something like a failed kotter pin? The fatal failure sounds a bit like Tony Bettenhausen Sr. but I thought the group here might have something more on point..

In 1961, Tony agreed to take a ride in friend Paul Russo's car. Before going on the track, the crew made an adjustment to the steering arm. That was done by removing the bolt at the forward end of the arm, making the change and putting it back. The mechanic put the bolt back in but did not replace and/or tighten the nut. Coming out of turn 4, the bolt fell out and Tony crashed to his death. The car left the track crashed through the fence and landed upside down before catching fire. I'm sure that is what the person you mentioned was talking about.

If the person was talking about Vukovich, they might have been thinking about the 1952 race when a kotter pin on the steering arm broke with 8 laps to go putting him into the turn 4 wall. He was not hurt and came back to win the next two 500s in the same car. Jim
